Deprivations of Liberty Safeguards (DoLS) requests

These forms are to be used if you believe that someone who is currently residing in a care home or, receiving treatment and care within a hospital are being deprived of their liberty and lack the capacity to make the decision about where to reside for the purpose of receiving their care and/or treatment due to a mental health impairment. When you complete the relevant form below, please ensure you have up-to-date care plans and risk assessments to support the request.
