NHS Health, Social Care and Family Services teams record information in IT systems and share information through such systems and other communications. We need to do this for the benefit of quicker access to accurate information so that you don't need to repeat your story. This also helps workers make referrals to other organisations and teams to co-ordinate responses for you or your family. So, your information may be shared with other people involved in your direct care, and that wherever possible, staff will tell you about this. You can change your mind at any time about sharing such information by contacting us.
